listing feedback: What are approriate/useful feedback questions?

Richard Iarossi had a featured blog yesterday on giving showing feedback. The comments were thought provoking and made me focus on the automatic feedback form that my MLS sends to us if we schedule showings online. I don't find the questions to be particularly relevant, but my broker has asked that we fill them out, so I do. There are 6 questions that are multiple choice with a yes/no/maybe or a scale of 1 to 5 and a couple of comments field -- one for the showing agent and one that the owner sees. MLS feedback form: 1.
