Got a hardship but did not complete the hardship affidavit to your servicer when applying for the HAMP (modification) program?  This is a required form and you MUST submit it to the servicer.

December 09, 2009: The servicer should still compile your file and send you the hardship affidavit along with other documents that require your signature and submission.  You must return this document along with the other requirements in order to be considered approved for the HAMP program.
