I think in our area we see more due to reasons 2 & 3.
Yes, I closed a HUD home last month that required four extensions ... one for a lender change, the others due to the lender not being responsive; they are a huge HUD lender, too.
Hi Renee and Aaron, we seem to have a lot of hick-ups right at the property inspection. The PCR reports often do not reflect the true condition of the property, and then the lender has problems.
