-Mortgage statement
-W-2 s and Tax return
-Pay stubs

 
 Any additional debt you may be carrying:
-Second Mortgage statements
-Credit Cards
-student loans car loans

Be sure to prepare a short statement about your current or future financial hardship situation....they will ask for this

Great advice Steve, people need to get out in front of the problem and stay there. This also is not for the weak at heart, so getting good advice and even someone to represent one in dealing with the difficult issues is a good path to take.
