To keep this simple it is a great idea to NOT name your pages index.html because you may wish to port your site at some point to .php or .asp or .cfm  or blend your html with dynamic pages  -   dont freak out if you have done this but it something to consider going forward

I just built a complete web site using blog software as the CMS and used descriptive folder names and link to everything with domain.com/folder-name/   and that way I can use serve anything as the default page in the folder rather than being locked into index.html -

 if you have thousands of backlinks to index.html pages then DONT delete them  as you can always serve other types of pages as .html but if you can avoid this from the beginning you will be better off....

I always use the kw in the page name if I can. However, I have seen sites that do well using the folder name as the kw then the page as index. Boils down to preference I guess.
