Water in Basement Duct Work, causes, solutions, problems?

The other day I had a home inspection done on a 3 level split home for a buyer who had an accepted offer on.  The inspector found water in 2 of the registers in the floor ----- the duct work in the basement runs through the concrete.  Any ideas of what might be going on?

 There is a drain tile system with sump pump.  That day the pump wasn't working right because the discharge hose outside was frozen so when it pumped the water it came back down.

One idea is that the water came back through drain tile and leaked into duct work since it is lower than the drain tile.

Two is that the hot furnace air is condensing in the cold floor

Three is there is a leak in the duct work to the outside water table/soil

Repair solutions are fill in duct work w/ concrete and replace duct work above, or drain tile duct work and furnace.

Is there any suggestions or really big problems?
