I hate Spam!!! Make sure your email is a "no show" so that it is completely invisible to spammers.
If You've "got mail" then "You've Got Spam!"
If you already have a form - even if you don't see the address - it may still be visible to the spammers. You have to look at the source code to find out.
I hate creating forms anyway. So I signed up for an account at emailmeform.com It's easy. It's free! You can have up to 5 different forms on different web pages. They do ask for a link from your page to theirs. If you need more than 5, then you can get a pro account for less than $10 per month.
Above is a sample form I created - and it has the image verification feature to prevent automated spam.
Sign up for a free account and log in. Then select create a new form from the nav bar you will see on the right. Follow the wizard and you will be done. You then cut and paste the code to your web page.
You can go to your account settings and choose to save a copy of the form to their server also. This is in case the form doesn't get thru to you for some reason.
You can also check you useage stats and see how many times a form has been submitted and how much spam has been blocked.
Always test your forms. Fill it out on your web page and submit - it should come directly to the email address you have specified. Nice.

Interesting tool here Eileen - thank you for sharing. I've used SpamArrest in the past with success, and I know there are other permission-based opt in/out programs as well.
Hi Troy; It prevents spam in three ways, One is by not having your email address visible anywhere on your website. Some forms may still have your email visible in the source code. (That's a techy one). The other way is the person has to type in the code to send. And last the service that hosts the form has spam filters that block any obvious spam from being sent to you.
Thanks for the info, Eileen. I have been looking for something like this. Everything I have found online was too hard for me to implement. I will give your suggestion a look.
Eileen, thanks for sharing this site. I just started using Google Docs . From what I can tell it works about the same. I will have to investigate emailmeform.com a bit more.
