VA Jurisdictional Addendum Following Forms Added: K1361- Disclosure of Dual Agency- Residential (07/12) K1362- Disclosure of Designated Agency ( 07/12) K1363- Disclosure of Dual Relationship-Commercial (07/12) Delete Form: K1206- Disclosure of Dual/Designated Representation DC Licensed REALTORS DCDE-District of Clombia Dept of Environment is now requiring " new" lead based paint disclosure. This new form provides an opportunity for owners to certify that they have no direct or first hand knowledge of lead base paint.
