Do You Have An Emergency Contact List?

I am curious, if you were in an accident would the first responders know who to contact?
Do you have a list of emergency contact names and phone numbers in your wallet?  
I do... It includes 4 names and is in my wallet and posted on my refrigerator.   It is printed on a bright yellow piece of paper.
If I am ever in an accident or something else should happen to me, I would want whoever was tending me to be able to notify the proper people.  First on my list is my brother, who is the executor of my estate.  
Noone thinks anything will happen to them, but it could!  My own Mother was hit by a car and died while crossing the street.  
I urge you to be prepared!
