Did you pay an "Assessment of Louisiana Citizens Insurance" fee when you renewed your homeowners insurance?  Everyone who paid this assessment is eligible for a refund of this fee.  Here's how to get it:
Go to the Louisiana Department of Insurance website at http://www.ldi.louisiana.gov/ and click on "Louisiana Citizens Insurance Tax Credit" on the right side.  Find and print the form titled "R-540INS".  Your refund request must be on this form or it will not be processed.  Send the completed form back to the LA Department of Revenue for your refund.
