The following is a copy of my intake form that I use for Staging. I generally keep this information in a clients folder on the left hand side. I also notate all communication between the client and on on the bottom of this form. Anyone is welcome to use it.

 

It is good to have an intake form because it keeps the client talking. If you have any questions or suggestions please feel free to post.

Lots of good info here - one thing I ask is how they were referred to us, and I have a place when I check off when I send thank you notes to the realtor, client and referrer.  (I do hand written notes.)  I would be very careful about writing the cc# on a sheet that is probably not kept locked up, since it is the primary contact form.  I have that information on a separate sheet so it stays in the file cabinet and never leaves the office.

Thank you for the feedback. We only keep this information in a safe place. It is in the same folder that contains our contracts. Ideally we are eventually going to find a database that will help us manage this information and our inventory.

I won't do a vacant staging that doesn't have electricity. All of the best staging in the world won't do a thing if you can't turn on the lights and see the house.

Linda; your form looks fine; but I do have one question. Do you really ask people for their PIN numbers and the last four digits of their social security number? And they give it to you? That seems a bit of a liability problem, and you don't need that info to process their cards.

Did you really mean the CVV/CVC 3 digit number on the back of credit cards? If so, that is fine. You just don't want peoples PIN numbers.

Yes, we mean the CVC code. We would only ask for the last four digits of their social if we are getting a verbal authorization to increase the amount of the estimate and they where not available to sign the amended agreement. This is so if they come back later on and say they did not authorize the increase we have verification information on file.

We also do not Stage homes that do not have electricty or running water. If there is no running water and someone uses the facilities that smelll may saturate our furniture which would be devestating.
