An Advance Health Care Directive is a legal document that provides directions for your health care if you are not able to speak for yourself or make decisions. The AHCD form is prepared well in advance to make sure that healthcare wishes of a person who is in a persistent vegetative state and is not able to make his or her healthcare decisions will be respected.

Problems arise, after the AHCD forms are filled out manually and stored in a safe place when they are needed if no one is able to find them or if there are multiple forms that do not clearly indicate the latest form that was signed by the patient reflecting patient's wishes. These problems point to the need for a central storage place, where health care providers can get quick and easy access to these documents, while maintaining privacy and confidentiality.

To overcome these problems Image-X with assistance from Cottage Health Systems Inc., Santa Barbara, California, created the electronic AHCD repository that allows the physicians to access the forms online. Moreover, California Governor Schwarzenegger signed the Assembly Bill 2805 (AB 2805) that permits Advanced HealthCare Directives to be digitally signed and notarized using the California digital signature standards which were established in law in 1995.

Image-X's solution electronically stores advance healthcare directives, and makes them available to physicians 24 hours/day via secure Internet or facsimile. The process of storing AHCD's also allows the system to generate an identity card for each patient. This "Orange Card" is mailed to patient that can be used by patient to access AHCD or allow the physician to acces the AHCD if patient or patient's agent provides the Orange Card to the physician.

Advantages of AHCD:

  1. Electronically stores the AHCD.
  2. Allows secure retrieval by physicians 24/7 in case of emergency.
  3. Can be filled online while maintaining privacy and confidentiality
  4. Ensures that the will of the patient is being met, ie. Patient autonomy is respected.
  5. Prints the Orange Card that provides a unique identifier for each AHCD for secure electronic retrieval.
