Has anyone been experiencing situations where the loan has been satisfied on a property but the lender has not removed the lien on the Deed of Trust?

Yes this happens often. Contact your closing attorney or title company and/or the Title insurance underwriter. It may take some leg work but the deed of satisfaction will need to be obtained and recorded.
