Pre-Kindergarten & Kindergarten Registration for Amityville, NY Schools

It's that time of year again...registration begins for the little people of Amityville, NY schools.

 

Dates & Times: April 4 & 5, 2011 from 9:30 am-3:00pm

                        April 4, 2011 from 5:00pm - 7:00pm

 

  • Children eligible for Pre-Kindergarten must be four years old on or before December 31, 2011
  • Children eligible for Kindergarten must be five years old on or before December 31, 2011

SCHOOL ADMISSIONS REGULATIONS

STATE LAW REQUIRES THAT THE FOLLOWING CREDENTIALS BE PRESENTED AT THE TIME OF REGISTRATION

  • Birth Certificate
  • All registrants must provide written evidence, signed and stamped from your PHYSICIAN  or HEALTH CLINIC that your child has been immunized against seven diseases:
  1.  
    1. 3 doses of Diphtheria (DPT)
    2. 3 does of Polio
    3. 2 doses of Measles for Kindergarten students, 1 dose of Measles for PK students. 1 dose on or after 1st birthday
    4. 1 dose of Rubella & Mumps
    5. 3 doses of Hepatitis B'
    6. 1 dose of Varicella or MD documentation of Chicken Pox disease.
    7. Haemophilus Influenzae type b (HIB) 3 doses if less than 15 months or 1 dose on or after 15 months for PK students
  • All students must have a comprehensive dental exam
  • Physical Examinations- Education law requires that all children entering school for the first time receive a physical examination. This should be done after Septmener 1, 2010
  • Two proofs of Residency for the Amityville, NY School District. Home owners: mortgage statement, tax bill, plus a Cablevision bill, bank statement, water bill, LIPA bill or phone bill. Renters:Current notarized lease or owner's affidavit with a copy of the owner's current tax bill, mortgage statement or deed to the property, plus any of the above mentioned bills.

Residents who are homeless should identify themselves as such at registration on the above dates. Some of the regulations above may not pertain to homeless students.

Grandparents/family members or guardians must bring evidence of legal custody before registration.

Only complete registrations will be accepted.
