I like the way you have set it up as a star rating - I have done a similar questionnaire but via email and they type in yes / no / maybe etc and ability to add notes.  It boggles my mind how so many agents don't return phone calls and are "bothered" when you ask for feedback

It boggles my mind as well at the lack of response we get. I give it three tries and a phone call to get feedback reports for our clients. How else can they know what buyers are thinking about their home?

With automated home feedback systems that have easy and quick forms, it's really a wonder why some agents wouldn't take the time to answer just a few of these.  Showing feedback helps your sellers realize what could be changed, whether it's something simple and cosmetic, to a price reduction.  If several agents or buyers are expressing the same concerns, the seller will (hopefully) realize there is an actual issue to be resolved.

Also, @Patty, you might want to try having a showing feedback request form like the customizable one we offer, which has a few quick multiple choice questions and an area at the bottom for additional comments or notes that the buyer or buyer agent would like to share.  You can choose which questions and answer choices to include in the form.  It's so simple to just click a few buttons and the home feedback request form automatically gets sent out as many times as you specify until the agent completes it.
