Reduced Cost Sports Physicals at Temecula Valley High School (TVHS)

Temecula Valley High School will be offering reduced cost physicals for only $40 for students interested in participating in team sports next year. This is the only time physicals will be offered by the district for our area High Schools. The Physicals are being organized by the San Diego Sports Medical Foundation (SDSMF). All proceeds will go to the purchase of supplies for the TVHS Sports Medicine Program.

The physicals are being held on Saturday, April 16, 2011 in the TVHS main gym.

A physical is mandatory by C.I.F. for summer camps, try-out and/or participation in all sports and it is a District Policy for participation on our Athletic Teams. If your student cannot attend the school physicals, he/she will have to see their private doctor for a physical.

Things to Remember:

1. Athletic Clearance Packet - Athletic Clearance packets must be down loaded and printed from the TVHS web site: tvhs.tvusd.k12.ca.us This entire packet needs to be completed and brought to the physical.
   
2. Please dress in Shorts and a T-Shirt.
   
3. Do not drink sport or energy drinks prior to the physical.
   
4. “CASH” is preferred or make checks payable to “SDSMF”

• 8:30am - 9am........... Last Name E - H
  
• 9am - 9:30am........... Last Name I - L
  
• 9:30am - 10am..........Last Name M - P
  
• 10am - 10:30am........Last Name Q - T
  
• 10:30am - 11am........Last Name U - Z
  
• 11am - 11:30am........All (Make-Ups)
