Notarized flip and signature pages for short sale!

Latest for me as a buyers agent in a short sale deal. The bank is requiring a copy of the GFE-seller concessions at closing, 2 pages - 1 is a flip affidavit, and 1 is a name affidavit......I guess there are more fraud instances than I realize! I have sold several short sale properties lately and this is a first.

ANybody have more details?
