Those are very serious questions and sadly are not discussed.  There are so many resources available.

There is a form called FIVE WISHES that is very useful - https://fivewishesonline.agingwithdignity.org

You might find it very useful when working with Boomers & Seniors - actually, it's an excellent form for everybody.  It's not just older folks that need to answer the questions you present.
