Does Anyone Use a 30 Day Survey?

I am thinking of incorporating one into my listing program and am wondering if others have gotten good response and/or good results from using them. I would think that it would bring any issues of concern to my attention and remind clients that I want to provide the best service possible to them. I would love to hear other's experience with them.

My broker sends one out for each transaction to the client. There is a rating on a scale of 1-5 on the experience, communication, process, etc. Then are some open ended questions as well. It's great to get feedback, plus some of them have make excellent testimonials! It's a must!
