Eyes (BLU-Blue, BRO-Brown, GRN-Green, GRY-Gray, HAZ-Hazel, MAR-Maroon, PNK-Pink). Hair (BAL-Bald, BLK-Black, BLN-Blond,, BRO-Brown, GRY-Gray, RED-Red or Auburn, SDY-Sandy, WHI-White). DOB-MMDDYY, POB-City+2letter state abb OR name of Country). Special procedures for the middle name, if you don’t have one. Enter NMN if No Middle Name whatsoever. If your middle “name” is only an initial enter it followed by MIO (middle initial only). For AKA women should enter any “maiden name”. All should enter any previously used names. That info should also contain any professional names a party is known by, such as an actor or singer, etc.

I strongly advise that you allow the fingerprint technician to substitute a “real” card to replace your printout. Let me stress: you should verify the above codes are correct for the agency that will be using the card. Additionally, only they have any “codes” that are required in other fields.
