Customer Satisfaction survey wanted!

Hi all,

Does anyone have a a good customer satisfaction survey you can share?  I am looking for something quick and to the point, yet effective.

Also, for those of you that have used customer satisfaction surveys, what is your experience with clients returning completed surveys?

I just completed a survey at a site called Surveymonkey.com and while they did have a professional version, they also have a free version.  I was thinking to myself that I might sign up to do a customer service survey.  You might want to check it out.

We have a written type of survey that we mail out with closing paperwork and a letter.  Every one I have ever sent out, has been returned.  It's a great way to know what your client's actually thought of you.  They can either sign or leave their name blank, but I usually have a pretty good idea who has sent them in.  Good Luck!

I was going to suggest making a form and putting it on your own site.  Surveys on other sites lend themselves to being used by other companies.  If you keep the data/survey on your own site, it is yours.
